Required: Date of Birth and USA Address.
Nationwide Criminal | Person & Address History | U.S. Sex Offender | FBI/US Marshals Lists | Terrorist Search/OFAC

CHOOSE: Online or Paper Authorization

ONLINE AUTHORIZATION:

1. Send the Applycheck Online Authorization Form link to your applicant.
2. Applicant completes the online form with Government Issued Identity Document (Driver’s License or Passport,etc.)
3. Applycheck sends you an email notification the report is complete.

PAPER AUTHORIZATION: 

1. Download the Applycheck Paper Employment Authorization Form.
2. Applicant completes the paper form with Government Issued Identity Document (Driver’s License or Passport,etc.)
3. Email the form to [email protected] with your Order #.
4. Applycheck sends you an email notification the report is complete.

NOTE:  This order requires your payment. If you would like the applicant to pay, click here and provide the link to your applicant. Tell your applicant which package you require and remind them to input your name and email address in the application so we can send the report to you.
